Community Navigation
Would you like to get more involved with activities, groups and services that are local to you and help you to improve or maintain your health and general wellbeing? Do you find it difficult to know where to start, face challenges to getting along to activities, or would you like some support to take those next steps?
SO:Linked is a service for adults who live in Southampton or who are registered with a Southampton GP. You can get in touch by phone, email, by filling in our self referral form below or by dropping by to one of our Come and meet us sessions across Southampton. One of the team will ask for a few details to ensure we give you the right support for you and will then explore with you what sort of activities or services you’d like to link with. We’ll then help you to make that happen.
Whether you’re managing a long-term health condition, facing challenges, feeling lonely or have had a change in circumstances; getting involved in community activities or volunteering can really help whatever your circumstances. In Southampton there are so many groups and activities – there really is something for everyone!
Contact us using the form below and a navigator will be in touch to chat through with you about your situation and help you think through what activities, support, advice or community groups you would like to be linked up with.
The navigator will be able to support you to take the next steps.
If you're looking for support with I.T and connecting with others through a laptop, tablet or another device please put that in the 'any other information' box. Please tell us if you have the device but need help using it, or if you don't have a device, or internet access.
If you're filling out the form on behalf of someone (making a referral) else please be sure to complete the any other information box and submit the form only once you have the person's permission.
Coronavirus (COVID-19) Update
SO: Linked have cancelled/suspended all events, meetings and drop ins. We are however still delivering a service which for the foreseeable future will focus on supporting Southampton through the coronavirus situation:
- navigating people who are affected by the coronavirus situation to practical and emotional support
- supporting/coordinating the Southampton voluntary sector response
All our drop ins, events and meetings are cancelled. We are focussed on supporting people through the coronavirus situation. Please continue to refer in the same way via the online form, email or phone number or EMIS / System 1 and please check the website and social media for regular updates.
If you or someone you know needs practical or emotional support at this time get in touch and we’ll do our best to link you to the support you need.
We also have a dedicated page which will be regularly updated with the Community Links for support during COVID-19 where you can get help and how you can be of help.
So if you are impacted by self isolation or other issues surrounding coronavirus contact us (details at the bottom of this page) and we will do our best to link you with voluntary organisations who can help in practical ways and offer emotional support. We can also signpost you to other resources and help you link in with virtual support groups, that can help you look after your mental health and wellbeing during this time.
NB: if you are making a referral on behalf of someone else you must tick the consent box in order for us to accept the referral.
SO:Linked offer a variety of weekly drop-in events across Southampton for people wanting to know a little more about our service and the diverse range of events, groups and volunteering opportunities across the City. They’re free to attend and there’s no need to book.
